With the increase in the elderly population all over the world, hepatitis B is becoming an important problem day by day. Especially the weakening of the immune system that occurs with aging, inadequate vaccination, low vaccine response rates compared to young people, physiological changes that occur with aging and the presence of accompanying comorbidities increase the burden of hepatitis B disease in the elderly. The risk of chronic liver disease and hepatocellular carcinoma (HCC), the most feared complications of chronic hepatitis B, increases with age. Therefore, elderly patients should be closely monitored for these complications. although the specific indications for treatment of chronic hepatitis B are clearly stated in current international guidelines, the management of chronic hepatitis B in elderly patients is complex. Physiologic changes and comorbidities that develop in old age complicate treatment decisions in patients. When starting treatment for patients, many factors such as patients’ physiological changes, accompanying comorbidities, compliance with and toleration of treatment, burden of treatment, and realistic treatment goals must be taken into account. The final treatment decision should be made between the clinician and the patient, taking into account the patient’s clinical condition. Vaccination is the best way to prevent hepatitis B. Vaccination of elderly patients at high risk is extremely important to reduce the morbidity and mortality caused by hepatitis B.
